Purpose:

To teach the Word of God to the children and to teach basic academic and extracurricular skills as subjects under the grand scope of theology

Ages: Rising Kindergarten (age during summer school) to 5th grade (grade in September of 2024)

Informed Consent and Acknowledgement

I, (parent/guardian of the above listed student), give my approval for my child's participation in any and all activities prepared by the 2024 CGS Summer School. I agree to pick up my student as soon as possible in the event of any of the following: sickness, violation or school rules/regulations

In case of injury to said child, I hereby waive all claims against the staff, teachers, and volunteers at the CGS Summer School. I waive my rights to take any legal action against this organization or the staff or volunteers. In the event of an emergency involving my child, permission is hereby granted to conduct any appropriate medical treatment deemed necessary by a licensed medical professional.


Student Media Consent and Release Form

Throughout the summer, students may be highlighted in efforts to promote CGS Summer School activities and achievements. For example, students may be featured in materials to increase public awareness of our school through displays, the web, and other types of media.

I, (parent/guardian of the above listed student), hereby give CGS Summer School and its employees, representatives, and authorized media organizations permission to print, photograph, and record my child for use in audio, video, film, or any other electronic, digital and printed media.
  1. This is with the understanding that neither CGS Summer School nor its representatives will reproduce said photograph, interview, or likeness for any commercial value or receive monetary gain for use of any reproduction/broadcast of said photograph or likeness. I am also fully aware that I will not receive monetary compensation for my child’s participation.
  2. I further release and relieve CGS Summer School, its Board of Directors, employees, and other representatives from any liabilities, known or unknown, arising out of the use of this material.
I certify that I have read the Media Consent and Release Liability statement and fully understand its terms and conditions.
